Virtual Class Waiver & Release Agreement

Please know that by booking and joining this class you represent and warrant that you are in good physical health and do not suffer from any medical condition which would limit your participation in the classes offered with Mahogany 360 Fitness, LLC. (It is your responsibility to inform the instructor of your limitations before class begins.) You understand that it is your responsibility to consult with a physician prior to and regarding your participation in any Boot camp classes, yoga, classes, programs, or workshops.

You understand the risks associated with the activities offered by Mahogany 360 Fitness, LLC., and you agree to follow all instructions so that you may safely participate in classes, workshops, or other activities. You hereby Waive and Release Mahogany 360 Fitness, LLC., from any claim, demand, cause of action of any kind resulting from or related to my participation in the programs offered and your presence on the property where the programs are held.

In taking part in the Boot camp classes, yoga classes, workshops, or other activities with Mahogany 360 Fitness, LLC., you understand and acknowledge that you are fully responsible for any risks, injuries, or damages, known or unknown, which might occur as a result of your participation in the classes, workshops, or other activities. You have read the above release and waiver of liability and fully understand its content. By booking the class you are voluntarily agreeing to the terms and conditions stated above.

 


 

Personal Training Class Waiver & Release Agreement

Because physical exercise can be strenuous and subject to risk of serious injury, we urge you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity. You agree that participating in physical exercise or training activities, you do so entirely at your own risk. Any recommendations for changes in diet including the use of food supplements, weight reduction, and/or body building enhancement products are entirely your responsibility and you should consult a physician prior to undergoing any dietary or food supplement changes. You agree that you are voluntarily participating in these activities and use of these facilities and premises, and assume all risks of injury, illness, or death. We are not responsible for any loss of your personal property.

You acknowledge that you have carefully read this “Waiver and Release” and fully understand that it is a release of liability. You expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action, and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the trainer or instructor for personal injury or property damage.

To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence.

If any portion of this release from liability shall be deemed by a court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.

By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.

  • Payments are due at the beginning of each month; Early payments are accepted.
  • Drop-In payments must be made before class.
  • No refunds after purchase.
  • All clients must have the Zoom app downloaded.
  • PLEASE DO NOT SHARE THE PASSCODES/PASSWORDS WITH OTHERS!!!
  • Stay visible during the class to get the full Virtual Gym Experience.
  • All clients will be Muted before class starts.
  • Clients will be able to enter 15 minutes before class to set up.
  • Clients will be able to join the Virtual Class if they are running late.
  • Clients can change their screen name, optional.
  • Weights and mats are required.
  • Kettlebells, dumbbells, weight bars, and even water bottles can be used as other options.
